When someone tells me they’re considering back surgery

My advice is:

They should wait.

98% of the time if they learned and practiced the right things they wouldn’t need to consider such an invasive approach.

It’s so painful and my MRI says I have a bulging disc

Check with your doctor that you don’t have and red flags for surgery. Data is sufficient now that says scans aren’t indicative of causes, rather correlations. 

Pain isn’t a signal that there’s tissue damage or that the disc is the cause. After thousands of surgeries we’ve realized that it’s better to really try conservative approaches first.

What conservative approaches are recommended?

Physical Therapy is probably the most popular. Chiropractic, massage therapy, yoga, strength training. Basically things that move you. 

In my experience this isn’t enough. As you can see from the beginning, “Try conservative approaches first” is probably the most vague answer, and that’s what’s given by most doctors. 

On the other hand, it makes sense. I don’t think any single profession has been able to replicate across the moajority of its practitioners consistency in helping to create good results. 

What is the problem?

The first problem in my opinion is that practitioners are more concerned about applying their technique than doing an assessment that gives them a good sense of what’s going on and how to move forward.

The second problem is that professions are not being updated to practice with science in mind. We need to work with a more holistic perspective. 

Why not surgery?

Surgery has worked very well for some people. But the data tells us that 85-95% of people in back pain don’t need it. 

Surgery also has the potential for complications either during, or after the surgery that can be life altering.

What would you recommend?

I always recommend an assessment before starting anything. I have created my own personal assesment called Insight. I created it because I saw the need to make sense of pain and symptoms for both myself and my potential clients. I feel that if I make an assessment and I can’t put good reason together for the symptoms then it’s unethical for me to work with the person. 

I’ve turned Insight into and educational and experiential assessment so that our relationship starts off in colaboration. I see myself less as an authority and more as a guide. My goal is to give authority back to them. I believe that should be the goal of every type of healthcare practitioner. 

Enlightenment and Empowerment

These are words my clients often use to describe their experience of the “classes” as they call their sessions. 

Their expereince is interactive,  functional, revidence-based, it’s about exploring, feeling, and getting to know. I believe that assessments should be on going. Transformation towards a goal should be agile. We do something reasonable based on evidence and review the results, and figure out the next best step. 

I believe clients should be given homework that is based on the session because change only happens when they can actively bring new patterns into their lives to disrupt old habits. Planning. to live on in the same patterns is planning to live in pain.
